Second Opinions Overview


The F&S network of subspecialty radiologists provides many consultations, over reads and second opinions for patients, referring physicians, other radiologists who have uncertainty about the original interpretation. Other reasons for second opinions include the following:

Ordering Second Opinion Consultations

Patients, imaging facilities, or physicians can send the original radiology report, the accompanying image(s), a completed F&S Second Opinion form, a Protected Health Information (PHI) form and payment to Franklin & Seidelmann via mail. Images should be provided on a CD-ROM.

Select the appropriate Second Opinions Order form below, print and complete the form and then send the images and payment to Franklin & Seidelmann to initiate your second opinion order. An F&S staff member will contact you directly to discuss details as needed.

If required, Franklin & Seidelmann can contact the original imaging facility to obtain the original report and images.

An F&S radiologist will interpret the study. A report will then be sent via fax, mail or web to the report requester and to the patient’s referring physician.

All patients requesting reports must be under the care of a referring physician who must be identified on the request form, in order to receive an F&S second opinion report.
