The F&S network of subspecialty radiologists provides many consultations, over reads and second opinions for patients, referring physicians, other radiologists who have uncertainty about the original interpretation. Other reasons for second opinions include the following:

Ordering Second Opinion Consultations

Patients, imaging facilities, or physicians can send the original radiology report, the accompanying image(s), a completed F&S Second Opinion form, a Protected Health Information (PHI) form and payment to Franklin & Seidelmann via mail. Images may be provided on a CD-ROM or hard copy film.

Select the appropriate Second Opinions Order form below, print and complete the form and then send the images and payment to Franklin & Seidelmann to initiate your second opinion order. An F&S staff member will contact you directly to discuss details as needed.

If required, Franklin & Seidelmann can contact the original imaging facility to obtain the original report and images.

An F&S radiologist will interpret the study. A report will then be sent via fax, mail or web to the report requester and to the patient’s referring physician.

All patients requesting reports must be under the care of a referring physician who must be identified on the request form, in order to receive an F&S second opinion report.

MRI Ankle 1 Report Type: MRI Ankle         Size: 68 KB This is a 51 year old female who first came to the office 03/04/2005 with right ankle pain. She stepped on ice the evening prior and twisted her ankle. PF's showed no frank fracture, dislocation or subluxations.

MRI Foot 1 Report Type: MRI Foot         Size: 68 KB Pain and swelling in the right foot.

MRI Foot 2 Report Type: MRI Foot         Size: 68 KB 49 year old female with ankle pain x one month, without a specific injury.

MRI Knee 1 Report Type: MRI Knee         Size: 108 KB This is a 53 year old female with left knee pain being evaluated for ACL tear.

MRI Knee 3 Report Type: MRI Knee         Size: 78 KB This is a 21 year old male with right knee pain after a twisting injury on 7/31/2005. Patient has had prior lateral meniscectomy in 2001.

MRI Knee 4 Report Type: MRI Knee         Size: 64 KB This is a 41 year old male with knee pain, mobility loss and swelling. The patient had a twisting injury one week ago on 08/05/2005. The examination was performed on 08/10/2005.

MRI Shoulder 1 Report Type: MRI Shoulder         Size: 84 KB 32 year old male with shoulder pain.

MRI Shoulder 2 Report Type: MRI Shoulder         Size: 104 KB This is a second opinion case. **Looking for fatty degeneration of rotator muscles.**

MRI Shoulder 3 Report Type: MRI Shoulder         Size: 64 KB This is a 51 year old female with left shoulder pain and restricted external rotation and abduction x 6 months. Received for second opinion.

MRI Shoulder 4 Report Type: MRI Shoulder         Size: 132 KB This is a 26 year old with a history of instability. Examination was performed on 12/20/2005.

MRI Shoulder 6 Report Type: MRI Shoulder         Size: 96 KB This is a 69 year old male with pain in the shoulder. Evaluate for rotator cuff tear.

MRI Shoulder 7 Report Type: MRI Shoulder         Size: Pain. Decreased Mobility for several years.

MRI CSpine 1 Report Type: MRI Spine         Size: 64 KB 57 year old male. Received for outside consultation is an MRI examination performed on 11/28/2005.
